A large organization working with a third-party vendor, partnered to improve quality care using a technology platform for patient navigation while patients had their services rendered at their medical facility. The organization invested into the platform and wanted to understand if the patients who utilized the platform were showing significant differences in cost and utilization when compared to patients who were not utilizing the vendor services?

Measure

Analytics consultants measured length of stay for patients by service line, discharge disposition, and cost savings for those patients who engaged with the third-party partner.

Outcome
  • $2.0M in cost savings with reduction in length of stay for those patients who utilized the platform at each visit prior to scheduled surgery and post discharge
  • 30% of discharge dispositions changed from long -term care facility to home health services
  • 1.5 day reduction in length of stay for patients utilizing the platform compared those patients who did not.
